Don't miss your chance to earn the 2022 Wellness Incentive. 

Participating employees can earn a Medical Insurance Premium Discount and participating spouses can earn a $250 HSA contribution by meeting ONE (1) body composition criteria and ONE (1) tobacco criteria:

1. Meet with your PCP. Visit must occur between October 1, 2021 and May 13, 2022.
2. Submit your results by May 13, 2022. You can upload a screenshot from your Physician Portal into the [Wellness Portal](http://verve.medikeeper.com) or have your physician complete the [Physician Letter](https://citizensenergy.sharepoint.com.mcas.ms/:w:/r/sites/HealthandWellness/_layouts/15/Doc.aspx?sourcedoc=%7BBAD216E7-3B33-4321-930A-66AABA83A93A%7D&file=2022%20Primary%20Care%20Physician%20Letter.docx&action=default&mobileredirect=true).  Your submission MUST contain the following information: 
     - Date of visit
     - Height
     - Weight
