Program Overview

We are excited to partner with you to assist you in achieving your health goals! By committing to the Diabetes Management program you understand the following:

  • I am committing to regular interactions with a healthcare provider. This may be virtual or in-person depending on the provider.
  • I may be asked to make a list of medications (prescribed, over-the-counter and herbal supplements) so my provider can assist with the best medication regimen and watch for drug interactions.
  • I am willing to share my blood sugar log/results with my provider and Northwind Pharmacy team with the goal of identifying opportunities and tips to help me feel my best.
  • I may be asked to have labs drawn, such as HbA1c, to assess my blood sugar control over the past 2-3 months, as well as to complete a urine test to be sure my kidneys are in good health.
  • I am committed to regular eye exams to help detect diabetic retinopathy and prevent major vision loss.
  • I will create healthy lifestyle goals aimed to ensure exercise, healthy eating and tobacco cessation (if applicable) are priorities.

In honor of your commitment to your health, Citizens Energy Group is providing to you:

  • Diabetes supplies (machines, lancets, test strips, syringes, etc.) with no out of pocket cost to you
  • Diabetes medications including both oral medications and insulins with no out of pocket cost to you

*It is important to note that the Diabetes Management program is voluntary, and at any time you can choose to withdraw from the program. In doing so your diabetes supplies and medications will be covered under the terms of your pharmacy benefit and may no longer be offered at no out of pocket cost.
