Onsite Goal Checks for the 2022 Wellness incentive will be October 17 - 18, 2022

Monday October 17, 2022

- Belmont: 7:30am - 9am  West Conference Room
- White River: 9:30am - 10:30am 
- Steam/Perry K: 11am - noon Steam Conference Room

Tuesday October 18, 2022

- Langsdale 1b: 7am - 10am  SWIRL Conference Room
- Corporate: 11am - 1pm  Wellness Office 2nd Floor

No sign-up necessary. If you cannot make any of these times, please reach out to Melissa Hehmann at [mhehmann@citizensenergygroup.com](mailto:mhehmann@citizensenergygroup.com) by October 5th to make other arrangements.

Who needs to do this:

- Those who received a goal at the Spring Biometric Screenings or when submitting your PCP Visit information 
    - If your BMI was greater than 30, your goal would be a 5% weight loss
    - If you were a nicotine user, your goal would be to ne nicotine free

Who does not need to do this:

- Those not participating in the Wellness Incentive Program, meaning you did not participate in the Spring Biometric Screenings or submit PCP Visit Information in May 2022
- Those who completed the Reasonable Alternative Options
- Those submitting PCP Visit Information confirming goal has been met
