Earn a Discounted Medical Insurance Premium

Don't miss your chance to earn the 2022 Wellness Incentive. 

Participating employees can earn a Medical Insurance Premium Discount and participating spouses can earn a $250 HSA contribution by meeting ONE (1) body composition criteria and ONE (1) tobacco criteria:

Criteria must be met either by visiting your Primary Care Physician (PCP) OR attending an onsite biometric screening.

Option 1: PCP VISIT

  1. Meet with your PCP. Visit must occur between October 1, 2021 and May 13, 2022.

  2. Submit your results by May 13, 2022. You can upload a screenshot from your Physician Portal into the Wellness Portal or have your physician complete the Physician Letter.  Your submission MUST contain the following information:

Option 2: ONSITE Screening

  1. Go to the Online Scheduler to schedule the location, day and time that works best for you.
