Important Information about 2020 Health Screenings
Please read this very carefully, as many commonly asked questions about health screenings can be answered by thoroughly reviewing this information.
All employees are encouraged to participate in the annual health screenings.
There is no charge for employees and spouses who are enrolled in Citizens’ medical plans.
A $55 fee will be charged to those who are not enrolled in the medical plan.
For everyone’s safety, health screenings will be appointment only.
No walk ins will be accepted.

Please note you will need to fast for 12 hours prior to your screening.
If you or your spouse cannot attend this year’s health screenings but would like the opportunity to qualify for the wellness incentives, please complete the tobacco affidavit and have your doctor complete the physician kit

WELLNESS INCENTIVES
This year, completing all the criteria for health screenings may be a challenge. This is why we are offering several reasonable alternatives to allow you the opportunity to still earn the wellness incentive.
Participating employees who meet the screening criteria, or have met the reasonable alternatives as outlined in the 2020 Wellness Plan, will receive a medical insurance premium discount.
Participating spouses who meet the screening criteria outlined, or have completed the reasonable alternatives, will receive a $250 HSA deposit into the employee’s HSA account
